Patient has foot pain following 2 foot surgeries. Patient Advocate helps the patient coordinate second opinions and treatment options for pain management. This included a referral to a pain management clinic, alternate pain management solutions, coordination of a wellness plan, and coordinating all health care team members.
Patient came to the office and during the health assessment done by the Patient Advocate, high blood pressure is detected. Patient advocate creates a home monitoring chart for the patient, and gets him a referral for possible medication management of his hypertension.
Patient injured his hand at work. It got infected and Patient Advocate helped him get a 2nd opinion, surgery and post op care. Patient Advocate created a question list for the patient to ask the ER doctors, surgeon, physical therapist and other health care team members.

Patient’s wife called Patient Advocate to help problem solve her husbands medical care. He had a simple hernia surgery that developed complications, and the patient ended up in ICU on a ventilator. Patient Advocate helped the wife organize his medical records, develop a plan of care, and researched alternative hospital care. Knowledge and organization gave her power in a very frustrating medical crisis.
